Historical Era: USA Pancreas Graft Duct Management


From 1987-95 over 90% of pancreas transplants were done using bladder drainage (BD). However enteric drained (ED) US pancreas transplant cases have significantly increased. For 1999-2003 US DD cases with a complete record for analysis, 83%SPK, 64% PAK and 51% PTA were done with ED (Figure 21). The use of a Roux-en-Y loop with ED has decreased in cases where duct management was reported: since 1996, only 29% have been done with Roux-en-Y. An advantage of ED is avoiding the chronic complications of BD that result in converting some pancreas grafts to ED. For 1999- 2003 BD transplants, 9% were converted to ED by one year and 13%-16% by 3 years (Figure 22).
