Historical Era: US Pancreas Graft Duct Management


From 1987-96 over 90% of US pancreas transplants were done using bladder drainage (BD). However enteric drained (ED) US pancreas transplant cases have significantly increased. For 2002-2003 US DD cases with a complete record for analysis, 82%SPK, 72% PAK and 57% PTA were done with ED (Figure 12). The use of a Roux-en-Y loop with ED has decreased in cases where duct management was reported: since 1997, only 29% have been done with Roux-en-Y. An advantage of ED is avoiding chronic complications of BD that result in converting some pancreas grafts to ED. For 2000- 2004 BD transplants, 9% were converted to ED at one year and 17% at 3 years.  After the first year posttransplant, the conversion rate was higher for PAK and PTA than SPK: 22% vs. 17% at 3 years (p=0.05).
